H&R Block's "Three Strikes, Your Out"

Today is my daughter's 29th birthday (Happy Birthday, lady!!).  She happen to bring up in conversation today about some possible mistakes made in their 2005 tax forms this year.  They had H&R Block do them this year.

Looks like she's not the only one with problems.  Both Lorie Byrd and Block themselves have had problems.

Anyone for a financial version of the "three strikes" law?
